I have a 'do not use - EVER' note in them (entered by me). I must have been pretty upset because usually I would reference more detail and/or order numbers with service failures...

Sorry I can't shed more light on the reasons...

They did 6 shipments Nov/Dec last year and did not make any appointments so we do not use them any more. This was LTL freight.
