See, that is the kind of enforcement that is truly necessary. After a serious incident the carrier is shut down until it is investigated. You can be sure that a good carrier will put procedures in place to ensure that this does not happen again. Either more training, more double checking, asking the driver to confirm the heights etc. There is lots that can happen but doesn't.
